Paperwork Requirements
Once registered, the online registration system allows you to print all required forms from the HOME page. Please be sure to download and print the following:

PRINT OFFICIAL “AYF PLAYER PACKAGE” from your Teamlinkt account (see document attached in your email once your registration is confirmed)

WHEN PRINTING THE PLAYER PACK (DO NOT PRINT PAPERWORK-DOUBLE SIDED)

PARTICIPATION, TRACKING and ID CARD FORM (Initialed and signed by guardian)- 2 pages

AYF CODE OF CONDUCT FORM (signed by guardian and athlete)- 1 page

WAIVER AND RELEASE OF LIABILITY- Minor (signed by guardian and athlete)- 1 page

MTBI/CONCUSSION Annual Statement and acknowledgement Form (signed by guardian and athlete)- 1 page

MEDICAL CLEARANCE FORM (Signed by PCP, MUST have a signature date for the year 2025) OR a DATED 2025 PHYSICAL- ONE of these MUST be completed prior to 7/28/25, please plan accordingly- 1 page

EMERGENCY MEDICAL TREATMENT, CONSENT and INFORMATION (signed by guardian)-1 page

IMAGE RELEASE-Minor (signed by guardian)- 1 page

ABSENTEE FORM (DO NOT fill out top part / DO NOT DATE, only sign by guardian)- 1 page
STUDENT REPORT CARD
Must be the end-of-year report card that schools issue in Jun showing all trimesters/semesters. This is not required for athletes entering Kindergarten in September.

BIRTH CERTIFICATE
Must Show original birth certificate and provide a copied birth certificate for Taunton AYFC to keep on file (original will be returned)
